Adult Health Promotion Clinic - HIV/AIDS

The Adult Health Promotion Clinic (AHPC.) is a federally funded clinic that is operated from monies given to the State of Georgia through the Ryan White Care Act Title II and Title III. The AHPC was created in 1992 through the South Health District to serve the 10 county area.



Case Management Services - Initial enrollment assessment into the program, assessment of client needs, schedule appointments, home visits and pharmaceutical assistance.

Clinic Staffing - Staffing for the AHPC includes a physician, nurse practitioners, registered nurses, a licensed practical nurse, behavioral health specialist, public health technician, case managers, social service technician, health educator, secretaries, dietitian and outreach volunteers.

Community Based HIV Services - AHPC-South holds HIV Support Groups for our area. For times and dates contact John Rogers at 229-245-8711 ext 222.

Drug Assistance Services - Assistance in obtaining medications through the State Drug Assistance Program (ADAP) or pharmaceutical companies as well as local resources.

Health Education Services - Basic and comprehensive information regarding disease progression, medication and management.

Housing Program - HOPWA (Housing Opportunities for People with AIDS) is a program administered by the U.S. Department of Housing and Urban Development (HUD) which provides funding support housing for people living with HIV and their families. Currently, this program is for short-term services and may be used for rent, mortgage payments or utilities bases on client eligibility.

Medical/Nursing Care Services - Physical exams, pap smears, collection of lab work and Tuberculosis (TB) screening.

Nutritional Assessment - Nutritional information is provided to promote adequate nutritional intake and weight maintenance. Nutritional supplements may also be provided based on individual client needs.

Outreach Services - Outreach workers provide consumers with appointment reminders, appointment adherence, medication adherence and outreach serves as a liaison between consumers and staff.

Physician Care Services - Evaluation of lab studies, assessment of medical problems and recommendations for drug therapy.

Referral Services - Referrals to specialists for medical and dental care as well as referrals to other social service agencies based on individual client needs.

Risk Reduction Counseling - Counseling to determine risk factors and risk reduction strategies. Pre and post-test HIV counseling provided.
